I am the parent or legal guardian of the player registered above and give permission for her to participate in the PMA Girls Soccer Clinic (July 27–31, 2026). I acknowledge that athletic activity involves risk of injury, and I release St. Pius X – St. Matthias Academy, the Archdiocese of Los Angeles, and its coaches, staff, and volunteers from liability for any injury or illness occurring during the clinic. In an emergency, I authorize clinic staff to seek medical attention on her behalf. I also grant permission for photos and video taken during the clinic to be used in PMA promotional materials.
