Billing Authorizationrequired

I authorize PMA to charge transportation fees to my student's tuition/account billing profile. The remaining monthly charges will be added August 2026 through May 2027. Transportation fees are contractual obligations and generally non-refundable.

Commitment Periodrequired

Enrollment is for the entire 2026–2027 school year. Transportation routes are budgeted and staffed based on annual commitments. Families who voluntarily withdraw remain responsible for the balance of their transportation commitment unless otherwise approved in writing by PMA.

Rider Expectationsrequired

Students must follow driver instructions, remain seated, treat others respectfully, and comply with all PMA conduct policies. Transportation is an extension of the school day. Violations may result in suspension or removal from transportation services.

Pick-Up & Drop-Off Responsibilitiesrequired

Students must arrive at least 10 minutes before departure. Buses will not wait for late arrivals. Parents are responsible for timely pick-up at designated drop-off locations.

Photo / Video Acknowledgmentrequired

Transportation vehicles may utilize audio and video recording systems for safety and incident investigation purposes.

Liability Acknowledgmentrequired

I understand that transportation services involve inherent risks associated with vehicular travel and agree to hold harmless PMA, the Archdiocese of Los Angeles, and their agents except in cases of gross negligence or willful misconduct.
